Berks food safety inspections found rodent and insect activity in the kitchen, dining, and bar areas of a restaurant

Berks County is divided into three jurisdictions: Reading, Muhlenberg Township and the rest of the county. Reading and Muhlenberg Township have their own food safety inspectors and the rest of the county is inspected by Pennsylvania Department of Agriculture inspectors.

The results will be posted every other week. All food vendors are inspected at least once a year. Other inspections can be triggered by a change of owner, a follow up if a facility had been out of compliance or by a complaint filed with the PDA.

If a food provider is labeled out of compliance, that means it has one or more violations that require a return visit by an inspector. The establishment is given time to fix the problem(s) and remains open for business.

Below are the results of inspections conducted between Aug. 2 and Aug. 15, 2023, that were filed in the state database as of Aug. 17 at 12:30 p.m.

Aldi #106, 20 Wilderness Trail, Hamburg, August 15: no violations.

Burger King #12924, 12 Jetson Drive, Hamburg, August 15: two violations. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in reach-in refrigerator equipment near drive through window. Drink ice bin at drive-thru window has dirt residue build up near top of bin at rear and also the ice machine in the rear storage room has a slime mold build up on top interior bin around chute area. Also some soda nozzles at customer self-serve fountain have a heavy residue build up.

Hamburg Market, 200 N. Fourth St., Hamburg, August 15: no violations.

Lou Ees Pizza, 500 Hawk Ridge Drive Suite 6, Hamburg, August 15: two violations. Refrigerated ready to eat, time/temperature control for safety food prepared in the food facility and held for more than 24 hours, located in the refrigerators and walk in cooler, is not being date marked. Some food items are datemarked and some are not. Wall at warewashing sink has taped coving that must be repaired.

Mountain Springs Camping Resort Snackbar, 3450 Mountain Road, Hamburg, August 15: no violations.

Olivet Blue Mountain Camp, 115 Mountain Road, Hamburg, August 15: no violations.

Taco Bell/Long John Silver’s, 65 Industrial Drive, Hamburg, August 15: four violations. The water temperature of the wash compartment of the manual warewashing equipment was 96°F, rather than not less than 110°F, as required. Soda fountain nozzles at drive through window area were observed to have heavy residue and was not clean to sight and touch. Observed fryers in the cook line area, with encrusted grease and soil accumulation. Grease dripping out of front of fryer. Also floors have spilled food debris/grease under fryer/grill area. The food facility does not have the original certificate for the certified food employee posted in public view.

Mancino’s Pizza & Restaurant, 7656 Lancaster Ave., Mount Aetna, August 14: four violations. Deli slicer has a string attached at knob holding blade guard that is not an approved cleanable material. Packing tape cover on stainless steel pizza bain marie must be removed to be easy to clean. Also cardboard on prep table shelf must be removed. Observed single-service, single-use articles stored in kitchen shelf area which were not in the original protective package or inverted. corrected. Toilet rooms do not have a self-closing door. Womens room door closure needs to be repaired also rear storage area bathroom needs a door closer.

Pilot Travel Center #518 (Flying J), 2210 Camp Swatara Road, Frystown, August 14: four violations. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Splash guard area behind soda nozzle dispensing area has old splash residue/mold build up. Observed clean food equipment and/or utensils in subway clean rack area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Corrected. Side doors located in the gas pump side area of the food facility has a large gap and does not protect against the entry of insects, rodents, and other animals. Drain lines and water lines under ice machine are leaking causing a water puddle and slime mold build up along pipes.

Tokyo Hibachi & Bar, 960 Woodland Road, Wyomissing, August 14: 20 violations. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Some food items in the walk-in cooler are stored open with no covering and under dirt hanging from the shelf above. Observed a plastic container without a handle being used to dispense soy sauce into small cups. Corrected. Scoop at the ice machine is being stored directly on top of the machine. Observed multiple foods stored directly on the floor in the walk-in cooler and freezer areas, rather than 6 inches off of the floor as required. Facility is using cardboard to cover the entire floor in the walk-in cooler. Thermometers for ensuring proper temperature of equipment are not available or readily accessible in on of the fish cases and one of the bain marie units at the sushi bar. Food facility does not have available sanitizer test strips to determine appropriate sanitizer concentration at the 3-compartment sinks. Facility also does not have a method to verify the final rinse temperature at the hot water sanitizing dishwasher. Mechanical warewashing equipment observed with build up of filth and food residue and not cleaned before use, and frequently throughout the day. Observed a hole in one of the lids being used to cover dry good storage containers in the kitchen. The following food contact surfaces were observed to have residue/dried food residue and were not clean to sight and touch: interior of the ice machine, a knife stored in a sheath on a hibachi cart, a metal scoop stored on the shelving at the end of the cooking line. The following areas have a buildup of dirt, grease, and/or old food debris: some of the shelving in the walk-in cooler, floor and equipment at the cooking area, floor and equipment in the hibachi area(s), floor and equipment at the sushi bar, and floor in the bar area. Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid. The handwash sink in the kitchen area was blocked by dishes and was not accessible at all times for employee use. Plumbing system not maintained in good repair – the cold water handle is missing at the handwash sink in the server area. Plumbing system not maintained in good repair – observed water leaking at the drain line for the 3-compartment sink and at the mop sink. Doors to the storage shed and back storage room were being propped open at the time of this inspection. Screening at the back entrance to the kitchen is damaged and does not protect against the entry of insects, rodents, and other animals. Food facility inspection indicates evidence of rodent and insect activity in the kitchen, dining, and bar areas. Facility’s pest control program does not appear to be sufficient. Potential rodent harborage areas on the exterior of the building perimeter in the back area due to supplies and other items being stored along the building. Working containers (spray bottles) in the women’s restroom and hibachi areas, used for storing chemicals, cleaners taken from bulk supplies, were not marked with the common name of the chemical. Food facility using loose rodent bait placed on the counter at the server station.Out of compliance.

Hazel’s Cafe At Mayfield On Noble, 544 Noble St., Kutztown, August 11: four violations. The food facility does not have the original certificate for the certified food employee posted in public view. The handwash sink in the kitchen area was blocked by a large container of sanitizer and was not accessible at all times for employee use. Corrected. Paper towel dispenser empty at the handwash sink in the kitchen area. Corrected. Mops are not being hung to air dry. Mop hanger is available, but is not installed on a wall.

Lyons Fire Company #1, 110 Park Ave., Lyons Station, August 11: two violations. Food facility does not have the proper type of test strips to determine appropriate sanitizer concentration. Facility has pH test strips and not sanitizer test strips. There is an accumulation of grease and old food debris on the floor under the fryers.

New China King Restaurant, 168 W. Main St., Kutztown, August 11: one violation. Mops are not being hung to air dry.

Northkill Rod & Gun Club, 1040 North Kill Lane, Bernville, August 11: no violations.

Shuberts Best, 5836 Four Point Road, Bethel, August 11: four violations. Prepackaged sandwiches, a refrigerated ready-to-eat time/temperature control for safety food, in the customer reach-in refrigerator area, was not compliant with datemarking by being labeled with a discard or use-by date of no more than 7 days. The water temperature of the wash compartment of the manual warewashing equipment was 68.9F, rather than not less than 110°F, as required. Facility still has no hot water available for cleaning food utensils or washing hands properly. The handwash sink located in the counter area does not have water at a temperature of at least 85°F. Out of compliance.

Berkshire C C 9th Green, 1637 Bernville Road, Reading, August 10: no violations.

Berkshire C C Club House, 1637 Bernville Road, Reading, August 10: 11 violations. Observed plastic storage containers and cardboard lining shelving in the dry storage room, which is not an approved material for food contact. Food residue observed on the can opener food contact surface. NOTE- PIC removed can opener from the food prep area to be cleaned and sanitized. Thermometers not observed in some refrigeration units in the prep area. Excess condensate pooled at the bottom of a reach-in refrigeration unit on the cooking line. Chlorine sanitizer test strips or test kit not observed to determine appropriate sanitizer concentration. Loose rubber door gaskets observed on the walk-in cooler. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 200 ppm, and not 50-100 ppm as required. Note- Ecolab tech arrived on site during inspection and adjusted sanitizer concentration during inspection. Static dust observed on fan cover and ceiling in the rear walk-in coolers. -Residue observed on floor in the rear walk-in freezer. Oil drums in the indoor refuse area observed full. Excess used cooking oil is not being stored properly in a receptacle with a tight-fitting lid or cover. Oil is not being removed from the facility at an appropriate frequency. Floor / wall / ceiling is damaged in some areas; not smooth and easily cleaned surfaces. Rodent droppings observed along floor perimeters in the kitchen area; behind and under equipment. Excess pooled water observed under the steam table and refrigeration unit in the prep area. Condensate is not being drained properly. -Grease and residue accumulation observed along floor perimeter behind the cooking line.

Observed four quarts and one pint of 2% milk offered for sale beyond the sell-by dates marked on the bottles (July 21-July 31). Corrected. Observed mold growth on one flavor of beef jerky pieces offered for sale at the front counter. Corrected by disposing of the jerky and cleaning the container. Observed a significant amount of dirt/sticky residue on the floor under the old soda racking unit in the back area. Walls in the back storage area near the 3-compartment sink have a black mold-like residue. Missing and damaged ceiling tiles in the 3-compartment sink area need to be replaced. Observed chemicals (multiple containers of motor oil) for retail sale displayed on shelving above cookies and potato chips. Corrected by relocating motor oil.

Chatty Monks Brewing Company, 610 Penn Ave., West Reading, August 3: four violations. Ambient air temperature of the reach-in refrigeration unit in the prep area observed at 49 degrees F. The handwash sink located in the 2nd floor bar area and 2nd floor restroom, does not have water at a temperature of at least 100 degrees F. Water temperatures observed fluctuating between 73-76 degrees F at the time of inspection. The handwash sink in the 1st floor bar area is not operable at the time of inspection. Employees do not have access to hand washing in the bar area. Soap and paper towels not available at the handwash sink in the basement prep area. Out of compliance.

PIC provided date labels. Pizza prep refrigeration unit observed inoperable. No food items observed being stored in this unit at the time of inspection. Grease accumulation and debris, observed along floor perimeters, behind equipment, in the prep area.
